REMOTE students:
    Print off the attached document.   If you do not have printing capabilities, there will be some copies available at the Packet Pick-up area.
       At the end of the month, send a photo of your completed card to dawn.smith@fpsk12.net  OR  rachelle.torres@fpsk12.ne OR you can drop it off at the Packet Pick-up area.

IN PERSON students: 
    You can print the attached document OR get a copy from your teacher.
    At the end of the month, you can return your completed card to your teacher.
