Farmington Public Schools' FHP1 Blue Cross Blue Shield Plan is administered by Michigan Employee Benefits (MEBS).
There is a $250 per person/$500 per family annual deductible that must be met before the insurance pays its portion. The benefit year is from January 1 through December 31 and takes effect your date of hire.
The FHP1 plan pays 90% of reasonable and customary office visits with a diagnosed illness, 100% of lab, x-ray, and most in-patient hospital charges.
Prescription coverage is provided through Caremark. Co-pays are $5 for generic medications and $30 for brand-name medications. Mail order is $10 generic and $60 for name-brand. For personalized prescription information you may register and log in to the Caremark website.

(FHP 2) BCBSM/MEBS
Farmington Public Schools' FHP2 Blue Cross Blue Shield Plan is administered by Michigan Employee Benefits (MEBS).
There is a $100 per person/$200 per family annual deductible that must be met before the insurance pays its portion. The benefit year is from January 1 through December 31 and takes effect your date of hire.
The FHP2 plan has a $10 office visit and urgent care co-pay as well as a $500 wellness benefit; other services are payable at 90% (the district will reimburse up to $250 per person/$500 per family towards percentage co-pays).
Prescription coverage is provided through Caremark. Co-pays are $5 generic medications, $20 for single-source brand-name medications and $ 30 plus a penalty for multi-source medications. Mail order is $5 generic, $20 single-source name-brand and $45 plus a penalty for multi-source name-brand medications. For personalized prescription information you may register and log in to the Caremark website.

Blue Care Network (BCN)
Blue Care Network (BCN) is an Health Maintenance Organization (HMO) Plan. BCN covers 100% of office visits and hospital stays and there is no deductible or co-pay. Subscribers must choose a primary care provider from the list of participating doctors. The prescription co-pay is $5 generic, $10 for preferred brand and $20 for non-preferred name-band medications. The mail-order co-pay is $10 generic and $20 name-brand.

FPS Dental (administered by ADN)
Farmington Public Schools has self-funded their dental plan. The name of your dental plan is "Farmington Public Schools Dental Plan". FPS does not process dental claims directly but, rather, has contracted out with ADN Dental (a large dental network in Michigan) to handle all of our dental claims processing. If you have questions concerning how a claim was processed please contact ADN directly at (248) 901-3705.
The FPS Dental Plan is a NETWORK
dental plan. Subscribers of the FPS Dental Plan have three dental networks
available to them: the ADN Dental Network, the Michigan Dental Plan network
and the Dentemax network. Employees will receive a higher level of coverage
when using an in-network provider. To find in-network dentists you can visit
the ADN
dental website (be sure to search each network separately using the
drop-down menu).

for specific information on your level of dental coverage.

NVA Vision (MEBS)
Your vision coverage is through National Vision Administrators (NVA) administered by MEBS. The vision plan is a network plan and you will receive a higher level of benefits if you use a participating provider. Please refer to the Schedule of Benefits for your level of coverage. Any questions concerning your vision coverage should be directed to MEBS at 800-968-6327.

Health & Dependent Care FSA Reimbursement Accounts (Group Associates)
The Health Care Reimbursement Account allows employees to save money on out of pocket medical, dental and vision expenses by having those expenses deducted from their pay on a pre-tax basis. The Dependent Care Reimbursement account allows employees to set aside money, pre-tax, to pay for eligible child care expenses during the year. Enrollment takes place each year in November for the following calendar year (or within 30-days of an employee’s date of hire). In some instances a “Qualifying Event” will allow employees to enroll mid-year. Please visit the Group Associates website for more information
